We are excited to sponsor our 2nd conference specifically for Direct Care Professionals (DCP). The 2023 conference was a huge success and we honored over 75 outstanding Direct Care Professionals statewide.

The focus of the DCP Conference is to show our appreciation for caregivers and the importance of their role in helping Medicaid participants to remain in the community. We will be accepting nominations for outstanding Direct Care Professionals that you as an agency feel would be deserving of attending the conference. Please complete the nomination form below and submit as many nominations as you would like.

The conference will be a celebration of the award winners including lots of praise, prizes, interactive sessions and a delicious dinner. We are also inviting a representative from the agency to attend the conference with the nominee’s that are selected. Please note that the conference is for the nominated DCP and an agency guest only (no additional guests of the caregiver(s).)

All celebrations will be held from 4:30 pm to 8:30 pm
